Just setting up iredmail for the first time.

Install went well - but had to start dovecot manually

anyway.....

trying to work out the settings for the plugin http://www.tehinterweb.co.uk/roundcube/#pisauserprefs

I have installed this - but in the config file it has this:

http://www.tehinterweb.co.uk/roundcube/#pisauserprefs

Which database do i need to point this too - or is userprefs not supported in iredmail?

iRedMail doesn't store per-user SA prefs in SQL database. So this plugin won't work with DEFAULT iRedMail settings.
You have to configure SA per-user prefs and make sure it's working, then configure this roundcube plugin.

Setting up spamassassin to use sql was very easy - sadly as its amavisd doing the scanning it ignores this. is it possible to tell amavisd to use spamassassin directly?

I didn't try it before, it's better to ask in Amavisd mailing list instead. Sorry.
