Simple: Gmail/Comcast don't use the same data as mail-tester.com to detect spam, they have their own system to detect spams.

Re: mail sent to spam despite mail tester 10/10

is it possible to know why the mail was sent to spam by looking into the header / subject /text of the message ?

You can check mail headers first, some servers will add related info in header (e.g. iRedMail), but not all mail servers do this.
