Is there anyway to have email considered spam by keyword globally? such as "breast implants", "Neurological disorder" and so on and so forth? Maybe I'm searching with the wrong phrases but cant seem to find a straight answer.

You have to define new rule and score in SpamAssassin config file (/etc/mail/spamassassin/local.cf):
