What is the process in e-mail flow in the following case?  A user with a local e-mail account which is set to relay messages, without keeping a copy on the local system, to another account outside the system (like an ISP, Google etc.).   In other words, are there any spam controls or is the message essentially relayed without checks?

Also, any difference if checking the keep copy on the system in the processing of email?

It's checked by Amavisd (+SA, +ClamAV).
