Optionally, optimize tables once in a while:
OPTIMIZE TABLE msgs, msgrcpt, quarantine, maddr;

I tried this and all mail (outbound and inbound) just started queuing and nothing was processing, until I rebooted the server.

Is this normal?

I didn't try it before, but i guess it causes database lock, or MySQL is too busy.
You should check MySQL state.
