If button 'Save copy in mailbox' was checked, that means it will forward to itself, and email address won't be displayed in below input box.

In forwarding, when you put an an forward email and click save copy in mailbox.  When email arrived, email cannot store in local mailbox. 

Solution
When you add the user's email address + the forward email address.  After save, it checked the save copy in mailbox but the user's email address will disappeared.
